In addition to our flagship program, Project Access, Central Plains manages the Coalition Test Project, which coordinates thousands of donated diagnostic tests for community clinic patients. Enrollment in Project Access is not required.
Additional past programs have included:
A hospital case management project that connected high utilizers of area emergency departments to primary care medical homes and provided education on the self-management of chronic conditions. This program reduced ED use by 53% by this patient population and achieved improved health outcomes.
The Enhanced Care Management program provided intensive, home-based care management services for chronically-ill Medicaid HealthConnect enrollees. Program participants experienced improved health outcomes and cost savings was achieved within the Medicaid program.
